Attention Coaches: This is mandatory for
all coaches including assistant coaches
MSA will not longer use the paper risk management forms as
we have in years past. Each year you will be required to
complete and online risk management form. Please read the
information below and click on the button to complete your
online risk management form.
1. This must be done prior to the coaches meeting - this is
a change from in the past
2. Since this will be the first time for all volunteers look
to the bottom of the page where it says "new user" - we are
aware of the fact you have no username and password - thus
"new user"
3. Every one must enter a valid e-mail address - if you
don't have on please use this one

4. The site will notify me when you have done your
registration - you do not need to send me your verification
- however please print it and keep it as it has your NEW
username and password
5. This is great because you no longer need to share
your personal identification numbers in an unsecure fashion
- ie the risk forms that passed through many hands
6. As always this will be done every Fall - you will not get
your referee check with out this background check
