been cancelled. If you paid with Credit Card
your credit card has already been processed. If
you paid by check we are in the process of
mailing refunds. We are very sorry for any
inconvenience this may have caused.

1. Signed North Texas Roster
2. Signed Medical
Releases (Do Not Have To Be Notarized)
3. Signed Guest Player Release Form for all
guest players playing with your team.
